ABSTRACT

Objective To investigate the value of ultrasound and contrast-enhanced ultrasound (CEUS) in the differential diagnosis of mass-type autoimmune pancreatitis (AIP) and pancreatic ductal adenocarcinoma (PDAC). Methods A retrospective analysis was performed for the clinical data, ultrasound findings, and CEUS findings of 11 patients with mass-type AIP who were diagnosed in Tangshan Workers' Hospital from January 2015 to December 2020, and their characteristic manifestations were analyzed and compared with the data of 23 patients with PDCA. The chi-square test was used for comparison of categorical data between two groups. Results For the 11 patients with mass-type AIP, CEUS had a diagnostic accuracy of 63.64%, and all of these patients had hypoechoic single lesions; the patients with clear boundaries, regular morphology, pancreatic duct dilatation or cutoff, and blood flow signal accounted for 54.55%, 63.64%, 18.18%, and 36.36%, respectively, while in the PDCA group, such patients accounted for 30.43%, 34.78%, 78.26%, and 21.74%, respectively, and there was a significant difference in the presence or absence of pancreatic duct dilatation or cutoff between the two groups( χ 2 =11.089, P 0.05). For the 11 patients with mass-type AIP, CEUS showed that 7 patients (63.64%) had hyperenhancement and 4 (36.36%) had iso-enhancement in the arterial phase, and 5 patients (45.45%) had hyperenhancement in the arterial phase and 6 (54.55%) had iso-enhancement in the venous phase; for the 23 patients with PDCA, 22 (95.65%) had hypoenhancement of lesions in both arterial and venous phases, and there were significant differences in the enhancement pattern in arterial and venous phases between the two groups ( χ 2 =30.345 and 30.084, both P < 0.05). Conclusion The enhancement pattern of CEUS and the presence or absence of pancreatic duct dilatation or cutoff have a relatively high value in the differential diagnosis of mass-type AIP and PDCA.

ABSTRACT

Objective To explore the relationship between the nicotinamide adenine dinucleotide phosphate (NADPH) oxidase p22phox subunit C242T gene polymorphism and large artery atherosclerotic cerebral infaretion(LAACI) among Han population in Songjiang district of Shanghai city.Methods The polymerase chain reaction-restrictive fragment length polymorphism analysis and gene sequencing techniques were adopted to conduct the gene type and allele frequency detection of C242T gene polymorphism loci in 310 Han patients with LAACI and 330 healthy individuals as control group in Shanghai Songjiang area.Then the correlation between C242T gene polymorphism and LAACI was analyzed.Results The level of MDA and the activity of NADPH oxidase in the LAACI group were significantly higher than those in the control group (P<0.05).The smoking rate,alcohol drinking rate,hypertension rate,systolic pressure,diastolic pressure,and levels of blood glucose,TC,TG,LDL,ox-HDL and ox-LDL in the LAACI group were higher than those in the control group (P<0.05),while the HDL level in LAACI group was lower than that in the control group (P<0.05).The frequencies of CT+TT genetype and T allelic gene in the LAACI group were much higher than those in the control group (P<0.05);the Logistic regression analysis found that smoking,hypertension,systolic pressure,diastolic pressure,blood glucose level,Lp (a),ox-HDL level,MDA level,NADPH oxiolase activity and NADPH C242T genotype were independent risk factors for LAACI.Conclusion NADPH oxidase P22phox C242T gene polymorphism has a correlation with LAACI and is another independent risk factor for LAACI except for hypertension,smoking and lipid oxidation.

ABSTRACT

Objective:To optimize the formula of adefovir dipivoxil tablets and investigate the dissolution in vitro. Methods:The formula was optimized by the D-optimal mixture design, the effects of the amount of filler ( X1 ,%) , the amount of disintegrant agent ( X2 ,%) and the amount of binder ( X3 ,%) were selected as the independent variables, and the friability ( Y1 ,%) , disintegration time ( Y2 , min) and dissolution of adefovir dipivoxil ( Y3 ,%) were the dependent variables. The similarity of the self-prepared prepa-ration and the reference preparation was obtained by using f2 similarity factor. The stability of adefovir dipivoxil tablets was evaluated preliminarily by high temperature, high humidity and strong light testing. Results:The optimal formula of adefovir dipivoxil tablets was as follows:the amount of lactose monohydrate was 67. 0%, the weight of croscarmellose sodium was 8. 0% and the amount of pregelati-nized starch was 12. 0%. The prepared tablets had lower friability, shorter disintegration time and higher drug dissolution rate. The dissolution similarity factors of the self-prepared tablets and the reference preparation in four dissolution media were all greater than 50. The results of influencing factor tests showed that the product should be moisture preservation. Conclusion:The formula of adefovir dip-ivoxil tablets optimized by the D-optimal mixture design is similar to that of the reference preparation, and the preparation process is feasible, which can meet the requirements of large production.

ABSTRACT

Objective:To prepare norfloxacin thermosensitive in situ gel and investigate its in vitro drug release behavior. Meth-ods:Poloxamer 407 and poloxamer 188 were used as the matrix of norfloxacin thermosensitive in situ gel,and the gel was prepared by a cold dissolving method. The formula was optimized by a central composite design-response surface method. In vitro drug release be-havior of norfloxacin thermosensitive in situ gel was studied as well. Results:Within a certain concentration range, the gelation tem-perature decreased gradually with the amount increase of poloxamer 407, and that increased gradually with the amount increase of poloxamer 188. The optimal formula was as follows:the concentration of poloxamer 407 was 20.6%(w/v),and that of poloxamer 188 was 5.7%(w/v),which obtained suitable gelling temperature. The release of norfloxacin from the thermosensitive in situ gel reached up to (87.5% ± 5.4% 7 in 6 h. Conclusion: Norfloxacin thermosensitive in situ gel has excellent temperature sensitivity, and can slow down the drug release,which shows potential use for vaginal drug delivery system.

ABSTRACT

Objective Pathologic complete response (pCR) has been suggested as a surrogate prognostic indicator in breast cancer patients treated with neoadjuvant chemotherapy.We assessed whether the likelihood of pCR and survival is associated with the immunohistochemistry-based molecular subtypes.Methods We retrospectively analyzed the records of l01 patients with breast cancer who received neoadjuvant chemotherapy between January 2007 and January 2010.Patients were dassified into four molecular subtypes based on the immunohistochemistry prnfiles of estrogen receptor,progesterone receptor,and HER2.Logistic regression was used to analyze variables associated with pCR.Results The pCR was achieved in 19 patients (18.8%).The triple negative subtype was an independent predictive factor for pCR (odds ratio,3.35,95% confidence interval,1.25-9.79,P =0.012),and the Her-2 subtype showed a trend for higher pCR rates (odds ratio,3.11;95% confidence interval,1.09-10.89,P =0.021) compared with the luminal A subtype.The pCR was significantly associated with prolonged disease-free survival (P =0.002).The triple negative subgroup had shorter disease-free survival (P =0.0006) and overall survival (P =0.008) than the other subgroups.Conclusions We demonstrated that the triple negative and Her-2 subtypes are more likely to obtain pCR when neoadjuvant chemotherapy is given,compared to the luminal A subtype.Despite the high pCR rate,the triple negative subtype showed worse survival outcomes,paradoxically,primarily due to patients who had residual disease.

ABSTRACT

Objective By using PEG-PLL and OPG-expressing plasmid to synthesize a nanocompound PEG-PLL-OPG (PPO), to investigate its effects on osteoblasts (OB) and arteriosclerosis (AS) in mice. Methods The incidences of osteoporosis (OP) and AS were studied in people. Collect people′s serum , and divide them into different groups According to the results of diagnosis, the patients were grouped into the groups of Control, AS, OP, and AS. PPO was used to deal with OB in different groups. The cell apoptosis, cell activity, the adhersion and calcification to scaffold PCL were determined by flow cytometry, MTT, alizarin red stain and SEM,respectively. PPO was also injected intp the ApoE-/-RANKL+/+ mice via caudal vein to demonstrate its effects on BMD and AS. Results PPO could increase the cell activity , inhibit OB apoptosis and promote cell adhersion and calcifica-tion on scaffold PCL in vitro. PPO could also decrease the area and calcification of atheromatous plaque , and in-crease the BMD of collum femoris in vivo. Conclusion PPO may be a new drug for OP and AS treatments.

ABSTRACT

High mobility group box l protein (HMGB1) is a highly conserved non-histone DNA binding protein,which is widely distributed in eukaryotic cells.HMGB1 is an important proinflammatory cytokine,involving in various pathological and physiological processes,such as inflammation,neurogenesis and angiogenesis.In the early stage of intracerebral hemorrhage,HMGB1 can induce secondary brain injury,such as inflammatory reaction,neuronal necrosis,and brain edema.However,in the late stage of intracerebral hemorrhage,HMGB1 can promote angiogenesis and neurogenesis,and thus improving neurological function.HMGB1 will become a novel therapeutic target for intracerebral hemorrhage in the future.

ABSTRACT

OBJECTIVE:To establish a method for determining residual solvents in Decitabine for injection. METHODS:Cap-illary GC method was adopted,and the contents of residual solvents were calculated by internal standard method. The column was capillary column Agilent DB-624,flame ionization detector was used with the temperature of 280 ℃ by temperature programming, and the temperature of injector was 230 ℃,nitrogen was used as the carrier gas,flow rate was 1.5 ml/min and the split ratio was 50:1,direct injection was used,and the sample size was 1 μl. RESULTS:The linear range was 0.100 3-2.507 mg/ml for ethanol , 0.101 3-2.532 mg/ml for ether,0.102 2-2.556 mg/ml for isopropanol,0.008 3-0.207 8 mg/ml for acetonitrile,0.012 4-0.309 3 mg/ml for methylene chloride,0.014 8-0.369 4 mg/ml for tetrahydrofuran,0.078 1-1.953 mg/ml for cyclohexane,0.024 9-1.250 mg/ml for toluene and 0.017 6-0.440 9 mg/ml for N,N-dimethylformamide(r≥0.999 2);RSDs of precision and reproducibility tests were lower than 2%;recovery was 96.70%-101.60%(RSD was 0.59%-1.82%,n=9). CONCLUSIONS:The method is simple,accurate and reproducible and suitable for determining the residual solvents in Decitabine for injection.

ABSTRACT

Objective:To establish an HPLC method for the determination of gabapentin and the related substances in gabapentin capsules. Methods:A Kromasil C8 column (250 mm × 4. 6 mm,5μm)was used. The mobile phase was methanol-0. 01 mol·L-1 po-tassium dihydrogen phosphate solution (adjusting pH to 6. 9 with 2 mol·L-1 potassium hydroxide) (40∶60). The detection wave-length was 210 nm. The column temperature was 40℃ and the injection volume was 20μl. The main component and the known impuri-ty were determined by an external standard method. The unknown impurities were determined by a self-control method. Results: The calibration curve of gabapentin was linear within the range of 2.5-20.0 mg·ml-1(r=0.999 9). The average sample recovery was 100. 74% with RSD of 0. 24%(n=5). Conclusion:The method is simple, accurate,specific and applicable in the determination of gabapentin and the related substances in gabapentin capsules.

ABSTRACT

Objective To investigate the correlation between the NADPH oxidase p22phox-A930G polymorphism and hypertensive intracerebral hemorrhage in Chinese Han population in Shanghai area.Methods The patients with hypertensive intracerebral hemorrhage and normal controls were enrolled.The polymerase chain reaction and the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) were used to detect the genotypes and alleles of NADPH oxidase p22phox-A930G.Results A total of 128 patients with hypertensive intracerebral hemorrhage and 151 healthy controls were enrolled.The levels of systolic blood pressure,diastolic blood pressure,glucose,and triacylglycerol,as well as the proportions of smoking and alcohol consumption in patients of the hypertensive intracerebral hemorrhage group were significantly higher than those of the control group (all P< 0.05).There were significant differences in AA,AG,and GG genotypes (42.2%,44.5%,and 13.3% vs.63.6%,27.8%,and 8.6%,x2 =12.757,P =0.002) and in A and G allele (64.5% and 35.5% vs.77.5% and 22.5% ;x2 =8.734,P=0.001) frequencies between the hypertensive intracerebral hemorrhage group and the control group.Multivariatelogistic regression analysis showed that systolic blood pressure ≥ 140 mm Hg (1 mm Hg =0.133 kPa) (odds ratio [OR] 13.952,95% confidence interval [CI] 7.242-26.879; P < 0.001),apolipoprotein A ≥ 0.99 mmol/L (OR 3.139,95% CI 1.012-9.733; P =0.048),and AG +GG genotype (OR 2.333,95% CI 1.253-4.342; P=0.008) were the independent risk factors for hypertensive intracerebral hemorrhage.Conclusions Among the Chinese Han population in Shanghai area,the NADPH oxidase p22phox-A930G polymorphism is an independent risk factor for hypertensive intracerebral hemorrhage.

ABSTRACT

Objective To investigate tumor lymphatic and mircovascular densities as prognostic markers in 69 cases of invasive breast cancer treated with partial or total mastectomy and lymph node dissection.Methods 69 cases of untreated primary unilateral invasive ductal breast carcinomas were selected.All cases were immunostained with D2-40 and CD31.Positively stained microvessels were counted in densely vascular/lymphatic foci (hot spots).The relationship between lymphatic vessel density (LVD),microvessel density(MVD) and prognosis was analyzed.Results The mean ± SD peritumoral lymphatic vessel density (P-LVD) was significantly higher than intratumoral LVD(I-LVD) (P < 0.01).There was a positive correlation of D2-40 LVD(peritumoral) counts with lymph node metastasis (P =0.003) and clinical stage (P =0.026),and CD31 microvessel density was found significantly associated with clinical stage(P =0.038).No significant association was found between above variants with I-LVD (P > 0.05).Univariate analysis showed that survival time was impaired by higher MVD and higher peritumoral LVD(P =0.007,P =0.008,P =0.014,P =0.024,log-rank test),but not I-LVD.Multivariate survival analysis showed that MVD,peritumoral LVD,TNM stage and lymph node metastasis were independent prognostic factors for overall survival.Conclusions Peritumoral LVD and MVD were significantly correlated with survival status of patients with breast cancer.This is the first attempt to predict prognosis of breast cancer patients by quantifying the peritumoral LVD and MVD.

ABSTRACT

Objective To investigate the expressions of hypoxia inducible factor-1α (HIF-1α) and vascular endothelial growth factor-C (VEGF-C) and the correlations with clinic parameters,and to make sure the effects of the two genes in the lymphangiogenesis and lymph node metastasis of breast cancer.Methods The expressions of HIF-1α and VEGF-C were investigated by means of immunohistochemistry in samples from 78 cases with breast cancer and 20 cases with breast benign lesions.The density of the lymphatic microvessels immunohistochemically stained by D2-40 antibody was calculated.The relationships between HIF-1α,VEGF-C,LVD and clinicopathological parameters were analyzed statistically.Results HIF-1α expression occurred in 52 out of the 78 tumor samples (66.67%),while VEGF-C expression was observed in 41 out of the 78 tumor samples (52.56%).HIF-1α and VEGF-C expressions in breast cancer were significantly higher than those in beingn disease (15% and 10% ; x2 =17.26,P =0.000; x2 =11.71,P =0.001).The expressions of HIF-1α and VEGF-C were significantly correlated with regional lymph nodal involvement (x2 =5.80,P =0.016; x2 =7.26,P =0.007) as well as late TNM classification (x2 =8.51,P=0.004; x2 =6.02,P =0.014),disregarding the patient's age,tumor diameter,histological grade and pathologic type (P > 0.05).In addition,HIF-1α expression was positively correlated with VEGF-C expression (r =0.254,P =0.025).Higher LVD was found in both high HIF-1α and high VEGF-C expression cases (t =2.19,P=0.017; t =3.25,P=0.001).Conclusion HIF-1α may play a crucial role in the lymphangiogenesis and lymph node metastasis by regulating the expression of VEGF-C in breast cancer.Therefore,HIF-1α may become a particularly promising target for controlling lymph node metastasis in breast cancer.

ABSTRACT

Objective To explore gene polymorphism of the C/T genotype of rs 16 944 in the promoter of IL-1β gene in male population living in the coastal area of Shandong,and thus to investigate the relationship between the gene polymorphism of IL-1β and gout.Methods A total of 276 gout patients and 268 healthy controls were enrolled.The possible association between the polymorphism of IL-1β-511 C/T and gout in Chinese was investigated and gcnotype frequencies and allelic frequencies were calculated by polymerase chain reaction (PCR) restriction fragment length polymorphism (RFLP) method.Hardy-Weinberg was used to verify the representativeness of the sample.Comparisons between the groups were performed with x2 test and t-test.Results There was no statistically difference in IL-1β-511 C/T genotype frequencies between gout patients and controls(x2=3.251,P=0.197,df=2).The allele frequencies of C and T in gout cases were not different from those in the controls (x2=2.941,P=0.086,OR=1.232,95%CI:0.971-1.563).Moreover,no association between IL-1β-511C/T genotypes and risk factors for gout were observed in gout cases by x2 test.Conclusion Results of the present study suggest that the C/T genotype of rs 16944 in the promoter of IL-1β gene is not associated with gout in male population living in the coastal area of Shandong.

ABSTRACT

Recent studies indicate that there are expressions of gastrin and its receptors in the tissue and cell line of colorectal cancer .Gastrin stimulates the proliferation of colorectal carcinoma as well as inhibits the apoptosis of colorectal carcinoma so as to promote the growth and invasion of colorectal carcinoma via a series of signal transduction pathways induced by its specific receptors. To inhibit the abnormally active signal transduction pathways that stimulate the proliferation of colorectal carcinoma will provide a new effective measure for the prevention and management of colorectal carcinoma.But the accurate signal transductions are not clear and need further research.
